Pre-Installation Checklist

Acknowledgement:

1. I agree to pay the remaining balance on the account or COD when the installers arrive on the jobsite with the materials prior to acclimation.

2. I understand that all Luxury Vinyl Plank (LVP) products require a minimum of 48-hour acclimation time in order to be properly installed, in accordance with ANSI and ASTM standards.

3. I acknowledge that I am responsible for moving any delicate or valuable items, as Floor Daddy, LLC does not move items such as pianos, grandfather clocks, pool tables, photo frames, or breakables. Floor Daddy, LLC shall bear no responsibility for any damages that may arise as a consequence, either to the premises or the materials therein.

4. I understand that scratches or harm to walls, moldings, and trim may occur due to the material's size and the installation process. This includes, but is not limited to, unintentional damage to all appliances, landscaping, gas lines, electrical wiring, plumbing, telephone installations, or personal property.

5. I acknowledge that installers shall not be held accountable for any harm to TV cables, telephone wires, alarm wires, water pipes, gas lines, or damage to water lines connected to toilets, appliances, sinks, etc. Floor Daddy, LLC does not engage in the disconnection of gas or water lines linked to appliances, and any associated liability is disclaimed.

6. I understand that I must vacuum carpets after installation and that Floor Daddy, LLC has discretion over seams, direction, and installation.

7. I acknowledge that improper cleaning and maintenance will void the warranty and that LVP flooring should be cleaned with a dry mop and approved cleaning solution. No wax should ever be used on LVP flooring as it will void any warranties, installation, and manufacturer included.

8. I understand that the use of rolling chairs without a fiberglass mat will void the warranty of the floor. If I am using roller furniture, I MUST use a fiberglass mat to protect the floor underneath, failure to do so will void the warranty.

9. I acknowledge that it is strongly recommended to install furniture pads to protect the floors for years to come. Floor Daddy, LLC will provide those to me to ensure that they are used.

10. I acknowledge that I have the right to file a written complaint with the Arizona Registrar of Contractors for an alleged violation of A.R.S. 32-1154, subsection A.
